Anyone can refer a potential new member to Thame and District Day Centre: a GP, district nurse, family members and, of course the potential client.
Once we receive a referral we will contact you to arrange to visit you at home to meet you to discuss your needs and assess whether we can meet them at the Day Centre.
We will then arrange for you to visit the Day Centre for the day. This will give us the pleasure of meeting you and you the chance to meet staff and other members. It will also help you decide whether or not you feel the Day Centre will suit you.
If you do decide to join as a member, and our assessment shows that we can meet your needs in line with the criteria set out by Social Services, we will offer you a place!
We carry out regular two-way reviews with our members to gather feedback on their experience and ensure that Thame and District Day Centre continues to support you and is able to meet your needs.
Click on the link provided below to download a new members Referral Form, which needs to be completed by hand then posted to us. Alternatively you or your representative can email or telephone us and we will help to complete this. All contact information is detailed in the ‘Contact Us’ page.
